Means the bone broke through the skin. It's pretty bad, i can't see him playing again. Shattered knee cap and a compound fractured ankle (same injury as Yow Yeah) he's unfortunately done as a player. He already looked a touch slower. He'll never have his zip again which is what made him so good.

Doesn’t necessarily mean blood pissing from the wound

It means the bone has broken through the skin which is usually accompanied by blood.
