I'm sure @Big Pete will remember what I'm talking about but I believe a player was seriously injured being dragged back and the ref didn't call held which was controversial and led to what we see in modern day.
My mind is thinking John Skandalis but I don't remember the exact incident only that it was a specific incident that caused the rule change.

What happened to the "onus is on the carrier to look after the ball" that they trumpeted this year. I have seen that strip get ruled the opposite at times this year
