Could add some variations to it when teams start figuring it out but at the moment it would be very hard to stop if Paix or whoever's playing 9 picks the right runner.

I guess it's on Paix to read the defence's motion and then pick the inside or outside runner. Maybe a wrinkle on the play that Mozer and BTK have done so well

That can open up another can of worms though with teams exploiting it and getting free interchanges.
Some teams already have phantom HIAs during games at certain times.

Potentially but nobody wants to have constant changing of wingers , centres and halves.
Also another thing the NRL needs to consider is when a HIA player comes back on the player who replaced him should come off.

It is indeed grim, so we have a dude who can't tackle, a dude who either knocks the opposition or himself out when he does tackle, a dude that can tackle but offers nothing in attack and finally a dude who can't catch the ball.
